It depends upon what is causing your thickened toenails. Here are a list of the usual suspects:

1- trauma- if you nail has been hit it can thicken up and come off/ grow out, or be thick for ever more

2- thick because of mechanical issues (clawed toes), age or poor circulation. A great 101 can be found here: Thickened toenails HQ

Once you figure out what has caused your thickened toenails then you can figure out the treatment for them.
