How can a get my custom orthotics top covers replaced?

Depends upon the orthotic. If the orthotic is plastic then the top cover is easier to replace. If it is a softer material then the top cover is slightly harder because you either have to stick one on the top or remove the old one, grind down any uneven bits and then replace.
